CONTINUING EDUCATIONAL UNITS
Ada Canyon Medical Education Consortium (ACMEC) - The Ada Canyon Medical Education Consortium designates the educational activity for a maximum of 5.75 category I credits.
Idaho Nurses Association (INA) - CEU accreditation has been applied for through the Idaho Nurses Association.
National Association of Social Workers (NASW) - CEU accreditation has been applied for through the Idaho Chapter of NASW.
Idaho State Bar (ISB) - CLEU accreditation has been applied for through the Idaho State Bar.

LATE FEE - of $25 will be charged for the registration applications received after October 1, 2005
CANCELLATION REFUND - requests must be made in writing to Brain Injury Association of Idaho. A fee of 25% of the cost of registration will be assessed for the paperwork and processing. No refunds will be given after October 1, 2005.
SCHOLARSHIPS - (Full or Partial) are available on a limited basis to assist survivors and family attending the conference.
ORGANIZATIONAL DISCOUNTS - 10% discount available off registration of "Professional Non-Member" price for organizations with 5 or more attendees.
Please note that a continental breakfast and lunch is included in your registration costs.

FUNDRAISER DINNER
We would also like to invite you to celebrate with us at our Fundraiser Dinner on Friday October 7th 2005. Food will be worth your while and jazz music will be provided by Northwest Nazarene University's finest. Tickets are available for an additional cost.

Directions to the Conference

From the Airport
Travel north on Vista Avenue for approximately 2 miles, you will loop around the historic Boise Train Depot, and will continue on as Vista turns into Capitol Boulevard. In less than a mile, you will turn left on Front Street and go one block to the corner of 9th and Front Streets. The Boise Centre on the Grove is on the right.

From I-84 West
Take 1-184, the Downtown Boise exit, for approximately 5 miles to Capitol Boulevard, turn left, go one block to Front Street, turn left, go one block to the corner of 9th and Front Streets. The Boise Centre on the Grove is on the right.

From Interstate 84 East
Take the Broadway Avenue exit, go right. As you travel north for approximately 2 miles, go left on Front Street. Travel on Front Street for a little more than a mile, at the corner of 9th and Front, the Boise Centre on the Grove is on the right.
